Onsite Pharmacists

At Well Care Health, our Pharmacists are onsite in various hospitals to assist during the discharge process of patients. They perform medication reconciliation, MTM, DURs, education training, prior authorization assistance, and ensure that most patients leave the facilities with their discharge medications, improving compliance and adherence.

Patient Follow-Up

Additionally, the team follows up with the patients within 48 hours, 15 days, and 30 days. These phone calls or visits, enable them to resolve any issues or questions patients may have regarding their medications. The goal of the program is to prevent any readmission within 30 days.

Reduce Costs and Readmissions

Nationally, Well Care Health is one of the few, if not the only program in existence that allow Pharmacists to perform and get compensated for clinical services. This program not only reduces hospitalization costs, it has proven to be effective in reducing readmissions and ultimately improving patient outcomes.
