Job Summary for Behavioral Health Housing Coordinator

The primary role of the Behavioral Health Housing Coordinator is in providing direct and support services to female or male residents in our BH housing program. Specifically, the 2 main functions are:

1) Supervising clients daily activities

2) Ensuring their participation in treatment related services.

NOTE: This Company is OPEN 24 HOURS: Must be available to work in varied shift, Holidays and on call.

Responsibilities:

  • Assist in supervision of residents activities and be aware of their whereabouts at all times within the facility

  • Attend all mandatory training.

  • Provide a brief overview of the housing program in regard to program rules, procedure, and services, with each new resident..

  • Maintains order and ensures compliance with rules and regulations; ensures that residents conduct themselves in an orderly manner.

  • Monitor and document clients for signs of medical distress and/or behavioral conduct on an ongoing basis.

  • Provide appropriate support and structure for residents and interacts continually with the residents.

  • Ensure wake-up and bedtimes, meal times, medication reminders, medical appointments, etc.

  • Enforces the housing program rules at all times.

  • Records accurate and up-to date reports.

  • Report to Supervisor any discrepancies in client's whereabouts and activities

  • Responds appropriately to all medical and other emergencies.

  • Conducts search of all residents bags when being admitted to the facility. Confiscates and secures any contraband found, and notifies the appropriate supervisor on duty.

  • Randomly inspects all rooms, common areas, and hallways every thirty minutes.

Qualifications:

  • High School Diploma required, Bachelor's Degree preferred, but not required.

  • Ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings.

  • Working knowledge of Google mail and it’s applications

Role and Responsibilities

  • Complete PAR’s (Prior Authorization Requests) timely and efficiently for Therapeutic and RMH services (additional PAR’s for services as assigned)

  • Track submission dates, expiration dates and other pertinent timeframes to ensure no lapses or gaps occur in services

  • Collaborate with physicians, advance practice nurses, psychologists, social workers, nurses, or other professionals to discuss service plans and progress.
  • Gather and maintain patient information and records, including social and medical history obtained via self-report, relatives, and other professionals.
  • Counsel patients during appointments as necessary.
  • Design/participate in the development of individualized service plans, using a variety of treatments.
  • Examine or order laboratory or diagnostic tests for patients to provide information on general physical condition and mental disorder.
  • Advise and inform guardians, relatives, and significant others of patient’s conditions and treatment when clinically appropriate.

Required Credentials

  • Graduation from a program specializing in a psychiatry/mental health;
  • ANCC certification in one of the following areas:
  • Psychiatric-Mental Health Nurse Practitioner
  • An unrestricted DEA registration in each state where direct services will be provided;
  • A state-issued Controlled Substance Registration (CSR) in each state where direct services will be provided (if required by state);
  • Registration with the state Prescription Monitoring Program (PMP) in each state where direct services will be provided (if required by state);
  • No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare, Medicaid, SCHIP)
