Looking for Providers who are currently enrolled in Medicaid with active NPI# QMHA (PSR) Independent contractor.

QMHA (PSR) Requirements: A bachelor's degree in psychology, sociology, social work, or related social services degree.

Ability to work independently and with individuals with mental health issues and their families. Attend weekly meetings and monthly training's.

Able to work variable hours including weekends.

Already enrolled as a QMHA with Medicaid.

QBA (BST) Requirements: Independent contractor.

Ability to work independently and with individuals with mental health issues and their families.

Attend weekly meetings and monthly training's.

Able to work variable hours including weekends.

Already enrolled as a QBA through Medicaid to provide BST.

All applicants MUST have: FBI Background Checks TB test results CPR and First Aid Certified Valid Driver's License Proof of Valid Auto Insurance Reliable Transportation

Primary Care Physician

Applicants should have an unrestricted Nevada professional license, and have completed training in a primary care specialty area.

--Primary Duties--

The staff provider is under the direct supervision of the Chief Medical Officer. Our providers are participants in, and leaders of, the health care team which works to support the health and care of patients at Well Care Medical and Behavioral Clinic

--Job Duties--

  • Provide direct patient care in accordance with standard medical practice.
  • Maintain current licensure and certifications as appropriate.
  • Coordinate Activities of clinic staff, as needed and as requested. Provide support, guidance, and training to staff as required.
  • Assist and work with other providers and staff to promote continuous quality improvement.
  • Supervise medical students, resident physicians and other health professionals at the health center as assigned.
  • Assist the Chief Medical Officer in establishing and implementing goals, objectives and policies and procedures of our community health center.
  • Ensure compliance with regulations, policies and procedures, and clinic standards of quality patient care.
  • Work all assigned clinic hours and meet any and all agreed-upon productivity expectations.
  • Complete all documentation, including patient charges, in a timely fashion, per clinic policies.

--Skills, Knowledge, Education, Training and Experience--

Required:

  • Must be licensed to practice medicine and prescribe medications in the state of Nevada.
  • Must meet current CME requirements for licensure.
  • Participates or eligible to participate with the majority of health care plans accepted at Well Care Medical and Behavioral Clinic.

Nurse Practitioner Specializing in Women's Services

The Nurse Practitioner (NP) is responsible for the assessment of patients in the designated service. The NP performs physical examinations, confers with patients in person and by phone/electronically, collaborates with others in the department to assure smooth flow of patient care consistent with established quality indicators and participates in the overall coordination of patient care.
Essential Functions:
• Practices under the standard procedure established for the NP role in accordance with requirement set forth by the Nevada Board of Registered Nursing.
• Conducts age appropriate assessments of patients, including history taking and physical examination consistent with department protocol.
• Performs assessment/data collection in an on-going systematic manner, focusing on physiologic, psychosocial, cultural, spiritual and cognitive status, patient/family education, family involvement, and patient advocacy.
• Consults with supervising physician or other clinicians as dictated by patient need and assessment findings.
• Participates with physicians and other clinicians in the department to assure effective patient care consistent with quality indicators for patients cared for in the designated service.
• Reviews medical record for follow up or ordering of diagnostic studies under established protocols.
• Establishes and maintains communication with other health care providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care.
• Establishes effective working relationships with members of the health care team, patients, and families.
• Delegates appropriately and coordinates duties/activities of health care team members.
• Evaluates effectiveness of care given by team members.
• Demonstrates knowledge of and applies safety principles as identified by the institution.
• Demonstrates responsibility and accountability for own professional practice.
• Participates regularly in staff development activities for unit and department personnel.

• Demonstrates knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care and unit functioning.
• Participates in unit and department Performance Improvement activities.
• Participates in nursing research activities as requested.
• Identifies and solves problems effectively.
• Demonstrates a service philosophy in all interactions with patients, families, and all members of the health care team.
• Demonstrates proficiency in the use of computers and telecommunication modalities to document, track and convey information.
• If practicing in a specialty role, fulfills the expectations established for the particular role.
• Perform other related duties as necessary.

Basic Qualifications:
• Minimum of six (6) months of experience as a nurse practitioner.
• One (1) year (within the past 3 years) experience as a NP providing care for women in family planning, perinatal care, menopause management.
• Master's degree in nursing
• Completion of master's program in Adult NP, Family Practice NP or Women's Health NP required..
• Must have a NPI
• Current DEA if required for specialty.

Mental Health Therapists provide assessment, diagnosis and treatment of individuals and groups with mental health, substance use and emotional/behavioral issues.

Positions Available: Full Time, Part Time or Per Diem

Qualifications: Licensed Clinical Social Workers; Licensed Marriage and Family Therapists; Licensed Clinical Professional Counselors; Licensed Clinical Alcohol and Drug Counselors.

Duties and Responsibilities: *Conduct psychosocial intake assessments and crisis/risk assessments and interventions with patients; *Conduct treatment planning, care coordination and discharge planning with patients; *Conduct individual and group therapy with patients; *Participate in Intervention Treatment Team meetings, Interdisciplinary Treatment Team meetings and staff meetings; *Interact and communicate effectively with therapists, case managers, psychiatrists, medical staff and administrative staff in order to provide effective and compassionate patient care.
